Enrollment checklist

Complete before your child's first day.

  • · Completed child information and admission agreement
  • · Parent/guardian contact information and work details
  • · At least two emergency contacts authorized for pickup
  • · Authorized pickup list with photo ID requirement acknowledged
  • · Emergency medical authorization signed and dated
  • · Current immunization record or exemption documentation
  • · Health statement or physician's statement as required
  • · Allergy, medication, and special-care instructions in writing
  • · Signed photo, video, and social media consent selections
  • · Signed acknowledgment of the parent handbook and policies
  • · Non-refundable $100 enrollment fee paid

Photo & video consent

Choose one option. Consent is voluntary, may be changed or withdrawn in writing at any time, and never affects your child's enrollment or care.

Option A — No photography or video
My child may not be photographed or recorded except where required for licensing or safety documentation.
Option B — Private classroom use only
Images may be used in private updates shared directly with our family and in internal program records.
Option C — Private family sharing group
Images may be shared in a closed group limited to enrolled families.
Option D — Public social media
Images may appear on Wild Arrow's public Instagram, TikTok, or Facebook accounts.
Option E — Marketing and website
Images may appear on our website, flyers, and promotional materials.

We never publish a child's full name, address, school, schedule, or location tags, and we remove content on request.

Emergency preparedness at a glance

Fire
Immediate evacuation to the designated outdoor meeting point with the attendance record and go-bag; roll call before re-entry.
Severe weather / tornado
Shelter in the designated interior room away from windows; account for all children; remain sheltered until the alert clears.
Lock-down
Secure entries, move children away from doors and windows, silence devices, and contact authorities.
Relocation
Move to the designated off-site relocation point; notify parents with the reunification location and procedure.
Medical emergency
Call 911, provide first aid/CPR, notify parents, and document the incident on an injury report.
Utility outage or hazard
Assess safety, relocate or shelter as needed, and notify families when care must end early.

Our go-bag holds the attendance record, emergency contact list, medical authorizations, medications, first-aid supplies, water, a flashlight, and a charged phone. Drills are practiced and logged, and parents are notified of the reunification location whenever an emergency requires relocation.
